Request a Basket

A Jack's Basket offers the gift of hope, love, and connection.

Celebrate a baby!

A Jack’s Basket is a complimentary welcome gift for the family of a baby diagnosed with Down syndrome. Baskets include baby gifts, a collection of books and resources, and the opportunity to connect with a parent raising a child with Down syndrome.

Who can request a basket?

Baskets can be requested from the time of diagnosis (prenatal or at birth) up to the baby’s first birthday by healthcare providers, parents, friends, family members, or a Down syndrome organization. Please select the type of request and provide as much information as possible to ensure delivery and parent connection (where available).

Baby's Information

We celebrate babies from time of diagnosis to one year old. For resources for children over one year, please check our Resources page!

This tells us which basket to send: boy, girl, or unknown.

When was the diagnosis of Down syndrome given? This helps us with parent volunteer selection, when available, and adjust basket contents.

This helps us adjust basket contents and match parent volunteers when available.

This helps us select a parent-volunteer to deliver the basket when available.

Provide additional specifics about Baby's Race or Ethnicity if you wish.

Where was, or will, baby be born? This helps us when a parent volunteer is available to hand-deliver the basket or when we look to see top hospitals where we might send brochures in the future.

Please feel free to share any additional information about baby. This info helps us provide extra resources and match parent volunteers when available.

We try to supply resources or a parent volunteer in the same language when available.

Frequently Asked Questions

Yes! Thanks to our generous donors, all baskets are complimentary, and are provided without charge to the requestor or the recipient.

All information exchanged is kept confidential and used to ensure the basket is given to the family in the most complete, specific, and timely manner – including making adjustments to the contents of the basket based on diagnosis, gender, or language and matching the best parent-volunteer when available. We respect HIPAA laws of privacy and no information is shared without parent consent.

We use trends around diagnosis, language, even hospital location, to adjust basket contents for this basket now, and for others later, as well as to build awareness of the program as we work to reach and celebrate every baby.

Whenever possible, baskets are delivered in-person by one of our trained volunteers who are parents of a child with Down syndrome. All other baskets are shipped directly to the family home and parents are offered the opportunity to connect remotely with a member of our parent volunteer team.

Unfortunately, we are unable to facilitate parent connections for families who live outside of the United States.

Each family who lives in the United States is given the opportunity to connect with one of our parent volunteers! Even if the family lives outside of the area of one of our trained volunteers, and are not able to receive an in-person delivery, we give every family the opportunity to connect remotely with one of our parent volunteers.

Unfortunately, at this time, we are unable to facilitate parent connections with families who reside outside the United States.

We ship to all 50 US states and all over the world! We do our best to ship internationally, but we are unable to cover any customs fees that may arise. Please note, we are not responsible for any lost/stolen baskets. If you do not receive your basket within two weeks of the request, please email info@jacksbasket.org.
