Deadline: Forms need to be completed by Wednesday, September 23rd. Items must be delivered to the Jack’s Basket office no later than Monday, September 28th. Please send a photo of your submitted item to events@jacksbasket.org. We will use this for promotional purposes.

Include any restrictions, expiration dates, or important details. Items must be redeemable for at least 12 months from the event date (October 9th, 2026).


Example: “Immediately,” “By September 15,” or “Flexible”

All donations will be packaged and displayed at the discretion of the Event Committee unless specific arrangements are made with the donor.
